Focused Care practitioners support our most vulnerable and complex patients. They work with both the patient and the clinical team to unpick barriers to care, guiding the patient to a place of stability.
Focused Care has its roots in Hope Citadel but is now an independently commissioned service in over 90 practices across Greater Manchester.
Shared Health Foundation
Shared Health Foundation is a clinically led organisation reducing the impact that poverty has on health.
They do this through a mix of advocacy for policy change, grass root level projects, and providing training to other professionals. They also run the Deprivation Focused GP Inequalities Training Scheme, and so provide us with our wonderful GP registrars.
The Deep End
The Deep End networks across the UK bring together medical professionals working in the most deprived areas.
Hope Citadel are proud members of Deep End Greater Manchester and the wider Deep End network.
